School-Based Health Center

Due to the temporary closure of APS schools, First Nations Community HealthSource’s School-Based Health Centers at Wilson Middle School, Grant Middle School, Emerson Elementary, Native American Community Academy, and College and Career High Schools will not provide on-site services. First Nations Community HealthSource, however, will be available to provide health and behavioral health services through telehealth and/or clinic appointments at our Zuni or Central Avenue health centers in Albuquerque.  If you/your child needs a medical and/or behavioral health appointment, please follow the steps below:

1. Call  505-308-8061 Monday – Friday between 8:30am-5:00pm to request an appointment OR complete an Appointment Request on this web page. Please make sure you indicate what type of appointment (i.e., Medical or Behavioral Health) you are requesting.
2. Complete and submit the Consent Form for the school that you/your child is enrolled. This form must be completed before receiving services.
